What is grief?

Grief is a natural process after a significant loss; however, when intense longing, deep sadness, and a feeling of emptiness persist for more than twelve months (six months in children) and impede daily functioning, we speak of Grief Disorder — also designated as Prolonged Grief Disorder in ICD-11 and DSM-5-TR.

Essential criteria include:

  • Persistent longing or yearning for the deceased person;
  • Intense emotional pain (deep sadness, guilt, anger, denial);
  • Avoidance of memories or, conversely, constant rumination;
  • Difficulty resuming meaningful activities, social isolation.

Risk Factors and the Maintenance of Grief 

  • A relationship of dependence or very close connection;
  • Traumatic or sudden death (accident, suicide, homicide, COVID-19);
  • Previous history of depression, anxiety, or trauma;
  • Limited support network or family conflict following the loss;
  • Criticism or stigma (“you should be feeling better by now”), which inhibit the expression of grief;
  • Existential crises about the meaning of life or shaken religious beliefs.

Most Common Symptoms of Grief

  • Cognitive: Recurring thoughts of "I could have done more," intrusive images of the moment of death, inability to accept the reality of the loss.
  • Emotional: overwhelming sadness, disproportionate guilt, anger, resentment, a feeling of disbelief or prolonged shock.
