Bipolar Disorder
What is low self-esteem?
Bipolar Disorder is a mood disorder characterized by alternating episodes of mania or hypomania (elevated energy and mood) and episodes of depression. According to the DSM-5-TR and ICD-11, it is mainly distinguished between two subtypes:
- Bipolar I: At least one manic episode (duration ≥ 7 days or hospitalization) that may be preceded or followed by major depressive episodes.
- Bipolar II: At least one hypomanic episode (≥ 4 days, without loss of contact with reality) and one major depressive episode; mania has never occurred.
Mood cycles vary between people: some experience acute phases spaced by long remissions; others have rapid cycles (≥ 4 episodes/year).
Causes and Risk Factors of Bipolar Disorder
- Genetics – Estimated heritability of 60–80 %; first-degree relatives have a seven times higher risk.
- Neurobiology – Dysfunctions in the limbic circuits (amygdala, prefrontal cortex) and in the dopaminergic, serotonergic, and glutamatergic systems.
- Environmental stress Significant life events (loss, jet lag, night work) can trigger episodes in predisposed individuals.
- Circadian rhythms – Disruptions in the sleep-wake cycle worsen mood instability.
- Substance use Alcohol, cannabis, and psychostimulants can trigger or intensify episodes.
Most Common Symptoms of Low Self-Esteem
Mania
- Abnormally elevated or irritable mood
- Inflated self-esteem or grandiosity
- Reduced need for sleep (≤ 3 h)
- Rapid speech, pressure to speak
- Grandiose ideas, unrealistic plans.
- Increased goal-directed activity or risky behaviors (excessive spending, reckless driving, unprotected sex)
Hypomania
- Identical symptoms but less intense, without severe social impairment or hospitalization.
Bipolar depression
- Persistent sadness, anhedonia
- Fatigue, psychomotor slowing
- Changes in appetite and weight
- Feelings of extreme guilt or worthlessness
- Thoughts of death or suicide
- Difficulty concentrating and indecisiveness
- Recognizing subtle changes early helps prevent symptomatic escalation.

How can we help?
Therapies with proven effectiveness
Pharmacotherapy (administered by a psychiatrist)
This includes mood stabilizers such as lithium, valproate, carbamazepine, and lamotrigine. Atypical antipsychotics such as quetiapine, olanzapine, and lurasidone are useful in manic or bipolar depressive phases. Antidepressants should be used with caution and only in combination with mood stabilizers to prevent manic switching.
Cognitive Behavioral Therapy (CBT)
It involves monitoring mood through diaries and arousal/depression scales, managing routines (sleep, eating, and exercise) to stabilize circadian rhythms, and challenging grandiose or self-deprecating thoughts, with an action plan for early signs.
Interpersonal and Social Rhythm Therapy (IPSRT)
It focuses on regulating daily routines and improving interpersonal relationships, and has been shown to reduce time to relapse in longitudinal studies.
Frequently Asked Questions
Have questions? We're here to help.
It is a chronic condition, but with appropriate medication, psychoeducation, and psychotherapy, it is possible to achieve prolonged stability and a high quality of life.
No. Abrupt discontinuation increases the risk of relapse or withdrawal symptoms. Any adjustments should be supervised by a psychiatrist.
Reduced need for sleep, increased energy, and racing thoughts are common prodromal symptoms. Documenting these changes helps in intervening early.
Studies show equivalent results in symptom management, adherence, and satisfaction, provided there is a trained team and a structured plan.
The psychologist will listen to your story, understand your goals, and explain how the treatment will proceed. This is a time to build trust and define the therapeutic plan.
